    Couldn’t you use the extra player as a sub? That seems to me to be the best way to handle an extra player.

    In theory, yes. But I’ve never really played a pick-up game where subs ended up working. Part of the experience, in my mind, is the disorganization of the game – no one is really in control, nothing is official…which is why it’s rarely the case that people want to take charge of actually dictating who comes on and off, and when.
    The other reason is that, 99 times out of 100, the “odd man out” is a latecomer, and it’s not easy convincing players who showed up on time to sit on the sideline for players who came an hour late.
    That said, you do make a good point – subs could work. It’s just that they haven’t in my experience.
